By law we are required to mandate that you use your legal/primary name when filling out the forms:

• Undergrad Certificate Program Application (fillable PDF)
  • This form is for non-matriculated students (not in a degree program). 
• Change of Major/Minor/College (Program Add/Change)-  Now an eForm in WISER! Change of Program Instructions 
• Certificate/Program of Study Add Change (etrieve)  
  • This form is for matriculated undergraduate students who would like to add/change a certificate or program of study. All other program adjustments are requested in WISER (see above). 
• CLEP Permission Form (see also CLEP Credit Policy) (fillable PDF)
• Credit Overload Request (fillable PDF)

• Re-Enrollment Application: (Scroll down to next section) We have different forms depending on your status when you left the university. Please select the appropriate form for your status. If you are not sure what Re-Enrollment Application you should submit, please email reg.forms@umb.edu include your full name and Student ID number , our Forms Team will direct you to the correct  application.

• Undergrad Transfer Credit Permission Form Note: Please browse the Transferology website for a list of the established UMass Boston transfer course equivalencies (fillable PDF)
• Withdrawal Form (DocuSign)
  >>Note: Use this form if you are a matriculated student looking for a university withdrawal or in semester withdrawal request.
  >>Newly accepted students please contact undergraduate admissions to defer or cancel your acceptance before the start of your first semester. 
  >>Guest Students who wish to withdraw please email registrar@umb.edu with the request.
