Graduate Student Forms
By law we are required to use your legal/primary name to process the following forms:
- Additional Program Form
 - En-Route Degree Application
 - Leave of Absence Request Form
 - Program Change Form 
- This form is for current graduate students (Masters & PhD) who wish to discontinue their current program and enter another
 
 - Program Fee Enrollment Form
- Most students can enroll themselves in Program Fee (CAS 600) via WISER self-service. Please review this help video on how to enroll. Note, the program fee enrollment form is for active students only. Inactive students must complete the RE-Enrollment Application below.
 
 - Re-Enrollment Application
- Re-Enrollment Application Deadlines:
>>Fall Semester: August 15
>>Spring Semester: January 15 
 - Re-Enrollment Application Deadlines:
 - Statute of Limitations Extension
 - Transfer Credit Approval Form (Graduate)
 - Withdrawal Form
- The withdrawal form is for current active students who wish to withdraw from their program. Please review the University Withdrawal Policy for more information. Newly accepted students please contact graduate.admissions@umb.edu to defer or cancel your acceptance before the start of your first semester.
